Replace or correct a Health Insurance Card
Procedure for making changes to your card following a change of identity
The Directeur de l’état civil du Québec will notify us of your change of identity. Once we receive the information, we will mail you the form entitled Identity Correction. You do not have to request it.
Step 1
Complete and sign the Identity Correction form that you have received.
Step 2
Go to one of our service points in the health and social services network (CLSC) with your photo, your form and 2 pieces of ID, including one with a photo.
Please note: Service unavailable at SAAQ service outlets
You cannot request to have the information on your Health Insurance Card corrected at a SAAQ service outlet since this service is not available.
Card delivery time
We will mail your new card within 2 weeks after we’ve received your complete application. This can take up to 3 weeks when we receive a high volume of applications.
