Pieces of ID

To renew your Health Insurance Card, you have to present 2 pieces of ID (including one with a photo) at a SAAQ service outlet or at one of our service points in the health and social services network (CLSC). The photo must enable us to clearly identify you.

Temporary measure to renew the Health Insurance Card

To renew your Health Insurance Card, you must mail your completed and signed renewal form to us, without going through the SAAQ service outlets. In most cases, you do not need to provide a photo: we will use the one on file.To find out more, read the news release.

Accepted identity documents

We only accept the valid or expired original of the following documents:

  • Québec Health Insurance Card or health insurance card from another Canadian province
  • Québec driver's licence or driver's licence from another Canadian province
  • Birth certificate or copy of an act of birth issued by a Canadian vital statistics office. For a birth in Québec, only the document issued by the Directeur de l'état civil as of 1994 is accepted
  • Passport
  • Certificate of Canadian Citizenship, or card with photo issued before February 1, 2012
  • Certificate of change of name issued by a Canadian vital statistics office
  • Certificate of change of designation of sex issued by a Canadian vital statistics office
  • Document issued by the Canadian or Québec immigration authorities

Identity checks

We may check your identity, for instance, when you:

  • renew your Health Insurance Card
  • request a correction to your identity on your card: first name, last name, date of birth or sex
  • request the replacement of a damaged, lost or stolen card
