Pieces of ID If you are between 14 and 74 years old and do not have a photo on your Health Insurance Card, you must make an appointment at a Société de l’assurance automobile du Québec service outlet or visit one of the service points in the health and social services network. You must present two pieces of ID, including one with a photo. The photo must enable us to clearly identify you. If you already have a photo on your Health Insurance Card, you only need to mail us your completed and signed renewal form.
