Request a review of a decision

You can apply for an administrative review if you disagree with a RAMQ decision. You have 6 months, from the date on which you are notified of the RAMQ decision, to do so. There is no cost involved.

Procedure

  1. Complete and print out the form entitled Application for Review.
    Specify what you wish to contest, including the facts and dates. If you need more space, use a separate sheet that you sign and date, and attach to the form.
  2. Attach any relevant documents (proof of purchase, proof of payment, medical documents, employment attestations, etc.). We recommend that you keep copies of all the documents that you send us.
  3. Mail your application for review to the following address:

Application for review for a wrongfully billed fee (accessory costs)

You must use a different form if your application for review concerns an application for reimbursement of a wrongfully billed fee: use the form entitled Application for Reimbursement (accessory costs) and check the box “Application for review”.

Analysis of your application

An agent may contact you to clarify your expectations and complete your file. The analysis of your application is based on the new information you provide and the documents on file for the entire period in question. If information is missing, this could delay the processing of your file.

After your file has been reviewed, the agent will notify you in writing of the decision rendered by the RAMQ following your application for review.

The RAMQ commitment

We undertake to process your application in keeping with our values: respect, integrity and equity. As specified in the Health Insurance Act, we have 90 days to render a decision following receipt of an application for review.

Deadline extension

After you have filed your application for review, you can provide additional supporting documents or comments. In this case, the Act provides that the 90-day time limit begins upon their receipt.

If we consider that additional information or documents are needed, the deadline will be extended by 90 days, in which case we will notify you accordingly.

If these deadlines are not met, you may wait for the decision to be rendered by RAMQ following your application for review, or contest the one already handed down before the Tribunal administratif du Québec (TAQ).

Contesting a decision before the Tribunal administratif du Québec

You may contact the TAQ if you believe that the decision rendered by RAMQ following your application for review does not respect your rights. You have 60 days, from the date on which you are notified of the decision, in which to do so.
