Replace or correct a Health Insurance Card

COVID-19: Please do not go to our Québec and Montréal reception desks, a SAAQ office or a CLSC for a Health Insurance Card

Special measures are in effect for the renewal or replacement of Health Insurance Cards. Thank you for not going to our Québec and Montréal reception desks, the Société de l'assurance automobile du Québec (SAAQ) or a CLSC for card-related services. You can request to have a damaged, lost or stolen card replaced. You can also request that the information it contains be corrected if there is an error or if your situation changes. A fee may apply.

The procedure for replacing or correcting a card is the same for your child.
