You may do so online using the Service québécois de changement d’adresse. Online The replacement of a Health Insurance Card online costs $15, payable by credit card (Visa or Mastercard). The replacement of a damaged card is free for persons age 65 or over, and for recipients of a financial assistance program with claim slips (carnet de réclamation). Replace your Health Insurance Card online Textual transcription Has your Health Insurance Card been lost, stolen or damaged? No problem. You can replace it online. But first, make sure you have a Government Authentication Service account. And if you’ve moved recently, double check that you changed your address, so your card is delivered to the right place. Then, click on “Apply online”. Identify yourself using the Government Authentication Service. Once you’re logged in, enter the required information. Select the person who needs to replace their card and the reason for replacement. Finalize your transaction and voilà, your request is sent! By mail Order a card replacement form online, or call us to obtain it. You will receive the form in the mail. Upon returning it, you will have to pay a $25 fee, either by cheque, bank draft or money order. The replacement of a damaged card is free for persons age 65 or over, and for recipients of a financial assistance program with claim slips (carnet de réclamation). The new card will include the same photo, signature and date of expiry as those on the card being replaced. Card delivery time We will mail your new card within 2 weeks after we’ve received your complete application. This can take up to 3 weeks when we receive a high volume of applications.
