Make my consent to organ and tissue donation official

There are 2 ways you can give RAMQ your consent to organ and tissue donation: you can enter your consent in the Registre des consentements au don d’organes et de tissus or affix a signed sticker on the back of your Health Insurance Card. Each donation may save up to 8 lives and help restore the health of 20 other persons.

Entering your consent in the Registre

You can enter your consent in the Registre des consentements au don d’organes et de tissus managed by RAMQ. Your registration will be valid for life and confidential. This way, your consent will be official and in keeping with your wishes when you die. Remember to notify your family to make sure that it respects your choice.

Persons allowed to download the form online

  • Adult 18 or older
  • Person aged between 14 and 17
  • Child aged from 8 to 17, registered by their parents or tutor

Child under 8 years of age and person under curatorship

You cannot use the online consent form and must call us to register if you wish to obtain a form for someone under curatorship or for a child under 8 years of age.

To register, download and fill out the paper form Consent to Organ and Tissue Donation. Then mail it to the address indicated on the form.

To download the form:

  • You will need your clicSÉQUR user code and password or you will have to create an account
  • You will be asked to provide additional personal information

If you are unable to provide the requested information or if a discrepancy is detected, you will be unable to continue the application online and will be prompted to call us.

Why authenticate yourself with clicSÉQUR?

Authentication tells us that you are indeed the person behind the request, and we then are able to securely send you a personalized form. This also makes it easier to enter it in your file when you return it to us.

Other ways of receiving the form

You will receive the form together with the renewal notice for your Health Insurance Card. You may also order it by calling us.

How the Registre works

The register contains over 3.3 million consents. When a person passes away, a doctor or an authorized person can quickly request that Transplant Québec or Héma-Québec check whether the consent is registered. To find out more on organ donation, refer to the website Signez don!.

Cancellation of your consent

You can cancel your consent at any time. Just call us or come to our Québec or Montréal reception desks to obtain the form Revocation of Consent to Organ and Tissue Donation.

Sticker on the back of the Health Insurance Card

When we mail you a renewal notice for your Health Insurance Card, you will also get a sticker. Sign it and affix it on the back of your card to give your consent to organ and tissue donation. You will have to repeat this every time you renew your card for your consent to remain valid.

Alternatively, you may order the sticker online on the website of Ministère de la Santé et des Services sociaux.

Living organ donation

You may donate an organ or portion thereof during your lifetime. Donation of a kidney or portion of your liver will help patients undergoing a transplant. To find out more, refer to the page Living Organ Donation on the website of Transplant Québec.
