Before getting started: how to register a family or a couple
Each person must complete their own registration form and provide their own supporting documents only.
For example, Juan, Yasmine and their child Ali, are registering for health insurance.
Here is how they must proceed.
Juan
Juan completes the Adult application form to register.
He identifies the members of his family.
He attaches his documents only.
He does not add the documents of Yasmine and Ali.
Yasmine
Yasmine completes the Adult application form to register.
She identifies the members of her family.
She attaches her documents only.
She does not add the documents of Juan and Ali.
Ali
Yasmine completes the Child application form to register Ali.
She states that she is the person in charge of the application and that Juan is the second parent of Ali.
She attaches the documents of Ali only.
She does not add her documents or those of Juan.
This allows RAMQ to create a registration file and produce a card for each person.
