List of documents confirming your coverage under your country’s social security plan – Newcomers

  • Belgium: Certificate of coverage under Belgian social security (form BE/QUE 104), to be completed by the competent authority before you leave for Québec
  • Denmark: Danish health card made out in your name (obtain it before leaving for Québec)
  • Finland: Certificate of coverage under Finnish social security (form SF/Q 4, FIN/Q 1, or F1/Q1) issued by the competent authority before you leave for Québec
  • France: Certificate of coverage under French social security (form SE 401-Q-207), to be completed by the competent authority before you leave for Québec
  • Greece: Certificate of coverage under Greek social security (form GR/QUE 5), to be completed by the competent authority before you leave for Québec
  • Luxembourg: Photocopy of your certificate of coverage under Luxembourgish social security (form LUX-QUE 3), to be completed by the competent authority before you leave for Québec
  • Norway: Photocopy of your certificate of coverage under Norwegian social security, issued by HELFO (health economics administration) or NAV (labour and welfare administration) before you leave for Québec
  • Portugal: Certificate of coverage under Portuguese social security (form POR/QUE 4), to be completed by the competent authority before you leave for Québec
  • Romania: Certificate of coverage under Romanian social security (form ROU/QUE 104), to be completed by the competent authority before you leave for Québec
  • Serbia: Certificate of coverage under Serbian social security (form SRB/QUE 13), to be completed by the competent authority before you leave for Québec
  • Sweden: Certificate of coverage under Swedish social security, issued by the competent authority and to be completed before you leave for Québec
