Organ and Tissue Donation

The Régie de l'assurance maladie du Québec offers you two effective ways to make known your consent to organ and tissue donation when you die. You have a choice.

The Registre des consentements au don d'organes et de tissus

You can officialize your consent by entering it in the register. To do so, fill out the form entitled Consent to Organ and Tissue Donation. The form, mailed along with Health Insurance Card renewal notices, is also available by calling us.

The Régie ensures that information entered in the register will remain confidential. When appropriate, Québec-Transplant and Héma-Québec will be able to quickly check, at the request of a physician or authorized person, whether your consent has been registered.

Even if you have given your consent to organ and tissue donation, you can change your mind at any time. The form Revocation of Consent to Organ and Tissue Donation is available from the Régie.

Sticker on the back of the Health Insurance Card

By signing the sticker and affixing it on the back of your Health Insurance Card, you can consent to organ and tissue donation when you die.

The sticker is sent to you along with your Health Insurance Card, for instance when you renew your card. You can also obtain the sticker at local community services centres (CLSC), hospitals and Services Québec offices.
