Lost, stolen or deteriorated Health Insurance Card
You can request the replacement of a lost, stolen or deteriorated card, whether for you or for your children under 18.

A fraudulent act involving the use of personal information concerning another person without that person's knowledge or consent., contact your local police department. You can also consult the website of the Consumer Measures Committee to find out what you can do if this happens to you.
Replacement of a Health Insurance Card
To have a card replaced, you can either:
- submit an online application. This is the fastest and cheapest way to obtain a new card;
- or call the Régie to obtain a card replacement form.
The new card will display the same photo, the same signature and the same expiry date as the card being replaced.
Replacement of a Health Insurance Card and driver’s permit
To replace both a valid Health Insurance Card and driver’s permit, you can either:
At a SAAQ service outlet
An additional fee applies for the photo and replacement of a driver’s permit.
Persons age 65 or over and recipients of last-resort financial assistance are exempted from paying the replacement fee for a Health Insurance Card.
The Régie undertakes to mail your card within 10 working days after receiving a complete application, as indicated in our Statement of Services for the Public.
If you still haven’t received your new card and require healthcare, contact the Régie.

Finding a lost card
If you find a lost Health Insurance Card, please drop it in a mailbox (no envelope). This way, it will make its way to the Régie.
