Lost, damaged or stolen card
The Régie de l'assurance maladie can issue a new card to replace a lost, stolen or damaged card. The new card will expire on the same date as the previous card.

Replacement of a lost, stolen or damaged card only
Call the Régie de l’assurance maladie du Québec or go to one of its reception desks during office hours to obtain the required form.
You will have to provide your signature and a photo meeting specifications established by the government. The section on photo and signature explains what to do and lists the acceptable pieces of ID. Some persons, however, are exempt from this procedure.
Fee
A fee of $23 is charged for replacement of a Health Insurance Card, over and above the amount payable for the photo. However, persons age 65 or over and recipients of last-resort financial assistance are exempt from paying the replacement fee.
Receipt of your card
The Régie will mail you a new card if you are still eligible for the Health Insurance Plan. In its Statement of Services for the Public, the Régie undertakes to issue your card within 14 days.

Replacement of a lost, stolen or damaged card and driver's licence
To have your Health Insurance Card and driver's licence replaced at the same time, you must follow the authentication process so that an authenticating official Authenticating official
Usually an employee of a CLSC, of the Régie de l'assurance maladie or of the Société de l'assurance automobile du Québec (SAAQ). can verify your identity.
The same photo will be used for both cards.

Only the photo taken at the service outlet can be used.
You will be asked to sign the authentication document in the presence of an authenticating official, who will send the Régie the information necessary for producing your Health Insurance Card.
Fee

The SAAQ will also charge a fee
for taking your photo and replacing your driver's licence.

Finding a lost card
If you find a Health Insurance Card, you must return it to the Régie de l'assurance maladie by dropping it in a mail box, without an envelope. The card will be automatically sent to the Régie.
