Usually an employee of a CLSC, of the Régie de l'assurance maladie or of the Société de l'assurance automobile du Québec (SAAQ). can verify your identity when you apply for registration for the Health Insurance Plan, renew a Health Insurance Card or replace a lost, damaged or stolen Health Insurance Card:
- Québec Health Insurance Card or health insurance card from another Canadian province;
- Québec driver's licence or driver's license from another Canadian province;
- birth certificate or copy of the act of birth issued by a Canadian vital statistics office. For a birth in Québec, only the document issued by the Directeur de l'état civil after 1993 is accepted;
- passport;
- Canadian Citizenship Card (wallet-sized with photo);
- certificate of change of name;
- certificate of change of designation of sex;
- Canadian or Québec immigration document.
