Government Authentication Service

To download the form, you must have a Government Authentication Service account. This account provides simple, fast and secure access to the online services of the Gouvernement du Québec. You will also need to provide additional personal information. If you are unable to provide this information or if it is inconsistent, you will not be able to continue. You will then have to call us.

You can:

  • use your existing Government Authentication Service account
  • create an account

If you have accessed the Société de l'assurance automobile du Québec (SAAQ) online services portal, you already have an account.

You cannot use the user code and password from a clicSÉQUR account to log in using the Government Authentication Service.
