Ostomy Appliances – Procedure to Follow

To obtain the first $700 amount, you must:

• complete the registration form;
• provide a medical certificate;
• send the form and the documents to the following address:

  Québec (Québec) G1K 7T3

Every year thereafter, the Régie will automatically send you the amount of $700 to cover the cost of replacing the ostomy appliances. Remember to notify the Régie of any change of address, since the Régie will send the payment to the address it has on file.

The medical certificate must attest to:

• the date of the operation;
• the type of operation (colostomy, ileostomy or urostomy) and its permanent nature;
• the insured person's full name.

The medical certificate must be dated and signed, and must clearly show the physician's name and professional number.
