Glossary

Authenticating official

Usually an employee of a CLSC, of the Régie de l'assurance maladie or of the Société de l'assurance automobile du Québec (SAAQ).

Calendar year

A year running from January 1 to December 31 of the same year.

Certified true copy

A copy of an original document that is certified true by a competent authority. Not just a photocopy.

Only certified true copies from the organization that issued the original document are accepted by the Régie de l'assurance maladie.

Consultation

An examination of a patient carried out at the request of another doctor.

Dependant

A dependant is a person:

  • under age 18;
  • domiciled with a permanent or temporary resident of Québec who exercises parental authority over him/her.

Someone in any of the three situations described below may also be considered a dependant if the person who has custody of him/her fulfills either of the following conditions:

  • the person is registered for the Health Insurance Plan as being in Québec for a temporary stay;
  • the person is a permanent resident of Québec and leaves Québec temporarily.

Situation 1:

  • The person does not have a spouse.
  • The person is age 25 or under.
  • The person is a full-time student at an educational institution.
  • The person is domiciled with a permanent or temporary resident of Québec who would exercise parental authority if the person were a minor.

Situation 2:

Situation 3:

  • The person does not have a spouse.
  • The person is of full age.
  • The person is domiciled with a permanent or temporary resident of Québec who would exercise parental authority if the person were a minor.
  • The person suffers from a functional impairment provided for in the Act respecting prescription drug insurance, and the impairment began before age 18.
  • The person does not receive benefits under a financial assistance program.

Detached worker

Workers carrying out activities in a different country than the one where their employer is located. They remain subject to their employer.

Diagnostic procedures

Procedures used by a doctor to evaluate a person's state of health and to determine the health problem in question.

Educational institution

A legal person or a private or public organization providing, at the elementary, secondary, college or university level, either general education or specialized training for a trade, vocation or profession.

Examination

Usually consists of a questionnaire and a physical examination designed to enable the doctor to evaluate a person's state of health.

Identity theft

A fraudulent act involving the use of personal information concerning another person without that person's knowledge or consent.

Solemn affirmation

(see form)

A sworn statement signed before a commissioner for oaths, who may be:

  • a mayor, councillor, clerk or secretary-treasurer of a municipality;
  • a court clerk or his/her assistant;
  • a lawyer;
  • a notary;
  • a justice of the peace;
  • the Secretary General, Assistant Secretary General or Assistant Secretary of the National Assembly;
  • any other person appointed as a commissioner by the Ministère de la Justice. For example, at the offices of the Régie and Services Québec, a commissioner for oaths is available during office hours. Most financial institutions have commissioners for oaths.

Spouse

Two persons (of the same sex or opposite sex) are considered spouses if they:

  • are married or have entered into a civil union;
  • have been living together for at least 12 months;
  • are living together and have had or adopted a child together.
