Replacement of a Lost, Stolen or Damaged Card and Driver's Licence

To have your Health Insurance Card and Driver's Licence renewed at the same time, you must follow the authentication process so that an authenticating official can verify your identity.

The same photo will be used for both cards.

What to do

You must:

Only the photo taken at the service outlet can be used.

You will be asked to sign the authentication document in the presence of an authenticating official, who will send the Régie the information necessary for producing your Health Insurance Card.

Fee

A fee of $15 is charged for replacement of a Health Insurance Card. However, persons age 65 or over and recipients of last-resort financial assistance are exempt from paying this fee.

The SAAQ will also charge a fee for taking your photo and replacing your Driver's Licence.

Receipt of your card

The Régie will mail you a new card if you are still eligible for the Health Insurance Plan. In its Statement of Services for the Public, the Régie undertakes to issue your card within 14 days.

If you are awaiting receipt of your card and require healthcare, please contact the Régie.

If other cards, licences or documents of yours have been lost or stolen, consult the Québec government website to find out how to have them replaced.
