Employment contract or attestation from employer

The document must indicate:

• the employer's name and the signature of an official;
• the employer's address (the address in Québec if the worker leaves Québec to work for a Québec employer);
• the beginning and end dates of the employment contract or, in the case of an attestation, of the employment period;
• the worker's identity.
