Contract between employee and non-profit organization, or attestation from organization

The document must indicate:

• the organization's name and the address of its head office in Canada;
• the beginning and end dates of the employment contract or, in the case of an attestation, of the employment period;
• the worker's identity;
• the place of employment;
• a brief description of the international aid or cooperation program and the alphanumeric code provided by the Régie de l'assurance maladie;
• the signature of an official of the organization.
